1. If employers have two corporate lines active, employers should follow these steps to activate the discount for all employees:
- Enter the following email address: Verizon@provista.com (case sensitive)
- Enter the default password: Discount1 (case sensitive)
- Click “Submit”
- On next page, click on “Access the Verizon Wireless Entity Agreement”
- On the Verizon Entity Program page, click on “Continue” at the bottom of the page
- On the Verizon Information page, fill out all the required information:
Members must have their Vizient Member ID, Zip Code and Federal Tax ID to proceed.
Note: 24-month term or extension required on corporate lines for all Members. Equipment upgrade eligibility date is not affected.
2. Then employees can access the discount once activated by employer here.

- Employees must register with My Verizon and enroll in paperless billing to retain 22% access discount, failure to do so will result in a 3% loss (down to 19%)
